O-RADS Ultrasound Lexicon

O-RADS US standardizes adnexal mass risk stratification to minimize unnecessary surgeries, categorizing lesions from O-RADS 1 (normal) to O-RADS 5 (>50% malignancy risk).

Key Discriminators:

  • Classic Benign (O-RADS 2): Typical dermoids, endometriomas, hemorrhagic, or simple cysts <10 cm (<1% risk).

  • Solid Components: Any papillary projection must be ≥3 mm in height to count.

  • Vascularity: Color Score (1-4) heavily dictates the final category; strong flow (CS 4) in a solid component immediately elevates risk.
