Signs of Pericardial Effusion on CXR

While a large pericardial effusion might present as a "Giant Water Bottle Heart" on a frontal radiograph, the lateral view may demonstrate the "Oreo Cookie Sign".

This sign is formed by a central opaque vertical line (the pericardial fluid) sandwiched between two lucent lines (the epicardial and pericardial fat).
