Stridor Imaging Signs

When imaging a pediatric patient with stridor, specific radiographic signs help differentiate the etiology.

Croup (laryngotracheobronchitis) typically shows the 'steeple sign' on a frontal (PA) neck radiograph, representing symmetric subglottic tracheal narrowing.

Epiglottitis is a life-threatening emergency that demonstrates the 'thumb sign' on a lateral soft tissue neck radiograph, caused by a thickened, swollen epiglottis.

Always maintain a secure airway when imaging suspected epiglottitis.
